Belhaven College Athletic Insurance
Policy
- All athletes at Belhaven College are required
to have primary insurance that covers athletic injuries in order to
participate in athletic practices or games
- Current/updated team rosters must be on file
with the Athletic Director’s office and with the Athletic Training
Department prior to any scheduled team activities
- It is the coach’s responsibility to notify
the AD office and Athletic Training Department immediately of
any changes in the roster
- The primary insurance policy must be valid and
paid for an entire academic year (August—May) in order to
participate in any athletic practices/games
- A copy/proof of this insurance must be on file
with the coach, the Athletic Director’s office, and the Athletic
Training Department
- In the event that the athlete’s primary
insurance policy lapses or changes, it is the responsibility of the
athlete to inform the Head Coach, Athletic Director, and the
Athletic Trainer
- Failure to inform Belhaven staff about
changes in policy may result in athletic injuries/illnesses that
are not covered by any insurance policy
- If an athlete enrolls in Belhaven without
primary insurance coverage, they must report to the Athletic
Director’s office in order to apply for health insurance
- This policy must be paid for in advance
for an entire academic year (August—May) in order to participate
in any athletic practices/games
International
Student-Athletes
- All international student-athletes must also
carry primary insurance that covers athletic injuries/illnesses in
the United States in order to participate in scheduled
practices/games
- A copy/proof of primary insurance must be on
file with Institutional Advancement (Becky Simms), the Athletic
Director’s office, and with the Athletic Training Department
- This policy must have with it an English
translation and include a evacuation and repatriation clause
- If an international athlete does not have
primary insurance coverage upon enrollment at Belhaven College, they
must report to the office of Becky Simms prior to participation in
any practices/games
- This policy must be paid for in advance
for an entire academic year (August-May) in order to participate
in any athletic practices/games
INSURANCE CLAIM FILING PROCEDURE
In the event that an athlete is injured and incurs
medical bills, the following procedure must be followed in order to
secure payment for unpaid bills:
- An athlete is injured and the athletic
training staff determines that other medical care is necessary
- The athlete goes to the referred
doctor/hospital with a copy of the athlete’s primary
insurance information
- The athlete’s primary insurance
information must be on file with the hospital
- All incurred bills must first be filed with
the athlete’s primary insurance company before the
secondary insurance company will process claims/bills
- Within 90 days of injury, an accident claim
form must be filled out and signed by both the athlete and the
Athletic Training Department
- It is the responsibility of the athlete
to get detailed, itemized bills and corresponding EOB’s (Explanation
of Benefits) forms to the secondary insurance company
- The contact information of the secondary
insurance company will be provided by either the Athletic
Training Staff or the Athletic Director’s office
- All questions regarding unpaid bills must be
addressed by the athlete with the secondary insurance company
Explanation of
Secondary Coverage
- The secondary insurance policy covers claims
up to the amount of $25,000
- The secondary policy has with it a variable
deductible. It is an “excess” policy
- A variable deductible covers costs
remaining after the athlete’s primary insurance company
has paid its predetermined percentage
- The amount remaining after primary
insurance payment will be paid by the secondary insurance
company as long as the injury is a result of athletic
competition/practice
