INSURANCE CLAIM FILING PROCEDURE

In the event that an athlete is injured and incurs medical bills, the following procedure must be followed in order to secure payment for unpaid bills:

  • An athlete is injured and the athletic training staff determines that other medical care is necessary
  • The athlete goes to the referred doctor/hospital with a copy of the athlete’s primary insurance information
    • The athlete’s primary insurance information must be on file with the hospital
  • All incurred bills must first be filed with the athlete’s primary insurance company before the secondary insurance company will process claims/bills
  • Within 90 days of injury, an accident claim form must be filled out and signed by both the athlete and the Athletic Training Department
  • It is the responsibility of the athlete to get detailed, itemized bills and corresponding EOB’s (Explanation of Benefits) forms to the secondary insurance company
    • The contact information of the secondary insurance company will be provided by either the Athletic Training Staff or the Athletic Director’s office
  • All questions regarding unpaid bills must be addressed by the athlete with the secondary insurance company

Explanation of Secondary Coverage

  • The secondary insurance policy covers claims up to the amount of $25,000
  • The secondary policy has with it a variable deductible.  It is an “excess” policy
    • A variable deductible covers costs remaining after the athlete’s primary insurance company has paid its predetermined percentage
    • The amount remaining after primary insurance payment will be paid by the secondary insurance company as long as the injury is a result of athletic competition/practice
